As a state-licensed childcare facility, we are required to adhere to all policies set

by the Department of Children and Family (DCF).

Temple Shalom Preschool is required to have the most up-to-date Florida Physical and Immunization forms on file at all times for each child attending our school.

When Temple Shalom Preschool requests a new health form prior to or by the expiration date listed on either form, it is your responsibility as parents to ensure it is submitted to the office on time.

If we do not have an updated form prior to or by the expiration date, your child will be required to stay home until we receive the updated forms.

 

A child cannot attend Temple Shalom Preschool without the most up-to-date health forms on file.

 

Medical exemptions are accepted with proof of paperwork. Religious exemptions are not accepted.
